Registered Nurse - Case Manager

The Registered Nurse Case Manager coordinates comprehensive patient care between the Wassaja Memorial Health Center, hospitals, specialty providers, home health agencies, IHS/PIMC, Tribal programs, and community resources. The position provides patient assessment, care coordination, discharge planning, follow-up, patient education, and assistance with healthcare access and insurance needs. The position also provides supervision and coordination of designated Community Health staff and supports Community Health, Purchase Referred Care (PRC), quality improvement, and accreditation activities.

KNOWLEDGE/SKILLS: • Knowledge of professional nursing principles, patient care, case management, care coordination, and discharge planning. • Knowledge of IHS, PRC/Contract Health, insurance, referrals, and healthcare resources. • Knowledge of community health and social service resources. • Knowledge of supervision, staff coordination, quality improvement, and healthcare policies and procedures. • Skill in patient assessment, education, care coordination, documentation, and follow-up. • Skill in managing multiple patients, referrals, providers, and healthcare services. • Skill in supervising and working collaboratively with healthcare and community health staff. • Skill in utilizing electronic medical records and standard computer applications. • Ability to communicate effectively and maintain professional relationships with patients, families, Tribal departments, healthcare providers, IHS/PIMC, and outside agencies. • Ability to maintain confidentiality and exercise sound clinical judgment within the scope of professional nursing practice. MINIMUM QUALIFICATIONS/EDUCATION: • Current, unrestricted Registered Nurse (RN) license in the State of Arizona. • Minimum of three (3) years of professional nursing experience in a clinical, hospital, home health, community health, or related healthcare setting. • Experience or training in case management preferred. • Previous supervisory or leadership experience preferred. • Experience with IHS, PRC, Tribal healthcare, or community health preferred. • Current Arizona driver's license and must meet Fort McDowell Yavapai Nation insurance requirements

ESSENTIAL FUNCTIONS/TASKS: • Coordinates patient care, referrals, specialty appointments, hospital discharges, home health services, and follow-up care. • Assesses patient needs and develops and coordinates appropriate care plans. • Conducts hospital, home, and community visits as needed to assess patient needs and continuity of care. • Maintains accurate patient records, obtains releases of information, and coordinates medical records with outside providers. • Provides patient and family education regarding treatment plans, medications, referrals, and available healthcare resources. • Assists Tribal members with insurance, claims, referrals, authorizations, and healthcare coverage issues. • Serves as an intake and coordination resource for the IHS Purchase Referred Care (PRC) program under the applicable 638 program requirements. • Coordinates PRC meetings and communicates outcomes with patients, providers, and appropriate Tribal staff. • Provides supervision and clinical guidance to designated Community Health staff, including Community Health Nurses, CNAs, CHRs, and Community Health Drivers. • Coordinates community health services, home visits, elder care, transportation, and other community-based services. • Serves as a liaison between the Health Center, Senior Center, Day School, Tribal programs, IHS/PIMC, and outside healthcare and social service agencies. • Participates in community health assessments, prevention programs, health education, and population health initiatives. • Participate in departmental performance improvement, quality improvement, clinical staff activities, and accreditation preparation. • Assists with developing and maintaining policies and procedures for Case Management, Community Health, and PRC/Contract Health. • Serves as a member of the Fort McDowell Yavapai Nation Child Protection Team. • Performs other duties as assigned or required within the scope of professional licensure.

$86,500.00 to $97,500.00 (DOE) annually
