Maintains a caseload of 12-15 with a minimum of 14 hours of service contact per month per client to include weekly face-to-face contacts with the client while promoting tribal wraparound philosophy and practices.

Assemble a Tribal WrapAround Team within two (2) weeks of referral by interviewing the client, identifying family members/natural supports/agency representatives and other significant persons. Uncover the client's strengths and needs, provides assistance with any immediate needs, as well as completes the initial Crisis Plan.

Within the first 30 days, coordinates the meeting of the Team and develops the initial individualized Plan of Care (POC) based on the identified strengths and needs, including a comprehensive 24-hour Crisis Plan. The Plan should reflect the best possible fit with the culture, values and beliefs of the client.

Monitor the provision and quality of services provided to the client and family through the Team and is the liaison when new services/resources need to be sought or developed.

Provides or arranges for transportation for his/her clients to appointments, crisis/emergency services, etc., if needed.

Completes all the necessary paperwork in a strength-based manner and maintains accurate information on-line, ensuring that demographic information is up-to-date, as well.

The Tribal WrapAround Care Coordinator is responsible for providing intensive care coordination for individuals, families, and youth who have been identified as being high risk for homelessness or is currently homeless. The care coordination will identify system partners such as Indian Health Services, Behavioral Health, Probation / Parole, Courts, Elder & Disability Services, Veteran Services, and Child Welfare to bring them together as tribal wraparound team to help empower the client toward self-sufficiency and independence. The position assists and aids individuals and families requiring assistance from the Tribe. The position fills a central role in linking homeless individuals and families to an appropriate housing plan of care and supporting them in tenancy sustaining services. The goal is to facilitate rapid and resourced exits from homelessness into permanent supportive housing and provides participant advocacy, case management, benefit establishment, linkage to Mental Health and/or Substance Use Services, linkage to stable housing and all other supportive services as needed.

The specialist's duties include utilizing the Placement Criteria Assessment and Referral Guidelines to assist citizens with their housing needs; this includes initial referrals and assistance with citizens in establishing benefits, accessing healthcare, insurance, scheduling of medical, dental, optical, and psychiatric services; coordinating with referral sources; completing orientation and intake; registering; and referring to other services that are more applicable to meet citizen's needs; and rapid rehousing services that includes emergency stabilization and managing and creation of the landlord network. The Intake Specialist assigns clients to appropriate services through housing including but not limited to: Homeless Housing, Assistance and Prevention (HHAP), Housing and Disability Advocacy Program (HDAP), Bringing Families Home (BFH), Behavioral Health Bridge Housing (BHBH), HomeSafe, and Native American Housing Assistance and Self Determination Act (NAHSDA). The specialist is required to compile and file all reporting statistics and funding reports in timeframes provided. This position directly supervises and manages as the live-in manager of Bi'du Khaale Community and their residents.

The Bi'du Khaale Resident Manager is an exempt position that serves as the live-in residential manager at Bi'du Khaale Development and provides prevention and outreach services through the Tribal WrapAround program. This position is a live-in residential manager at Bi'du Khaale Community and often consists of irregular business hours and may require work in early mornings, evenings, and weekends. May be required to perform after-hours standby services as needed or some evening work, especially during the winter season.

The Bi'du Khaale Resident Manager is an exempt live-in position that resident manager and prevention and outreach services. The resident manager is responsible for upkeep of the Bi'du Khaale that consists of grounds maintenance; housing maintenance; minor carpentry, plumbing, HVAC, and electrical repairs; bidding, scheduling, and overseeing for carpentry, plumbing, HVAC, and electrical projects/repairs. The position requires community outreach and prevention programming to occur onsite and at times off-site locations. These services include community center programming, food sovereignty educational and prevention events, and community outreach and engagement.

Support logistics for EcoCultural burning including preparing participant rosters, printing burn paperwork, coordinating food and bathrooms

Ensure accurate post-program/ training documentation

Gather participant info and draft Training Stipends in protocol with TERA financial procedures

Support Program Manager with outreach by distributing flyers and sending outreach emails

Set up and manage nursery for Tule starts

Support office upkeep, cleaning, and maintenance as assigned by the Program Manager

OPTIONAL - Participate in EcoCultural stewardship work with TERA crew up to 2 days per month if desired by the selected candidate
